![]() For the convenience of our parents and students, we have posted all the necessary forms you'll need to re-register for the upcoming school year. Just click on the links at the right of this page to open, download, and print the forms required. Complete the necessary forms and return to the high school office or e-mailing them to: cdubois@fhusd.org. (Do not e-mail the Off-Campus Permission Form as it requires a parents' signature, in the presence of office personnel, or must be notarized and returned to the office in hard copy form.) You can also pay for any academic, activity, or athletic fees online (at the bottom of this page). Be sure to include your student's name in the comments area of the checkout form when you pay fees online.
