For the convenience of our parents and students, we have posted all the necessary forms you'll need to re-register for the upcoming school year. Just click on the links at the right of this page to open, download, and print the forms required. Complete the necessary forms and return to the high school office or e-mailing them to: cdubois@fhusd.org. (Do not e-mail the Off-Campus Permission Form as it requires a parents' signature, in the presence of office personnel, or must be notarized and returned to the office in hard copy form.)

You can also pay for any academic, activity, or athletic fees online (at the bottom of this page). Be sure to include your student's name in the comments area of the checkout form when you pay fees online.

If you are new to our district, you will also need information for enrollment or immunization requirements (available on our district Web site).



Fees are charged to maintain programs and offset costs and they are as follows:

General Fees

  • $75 Parking fee (per vehicle used on campus) 
  • $30 Athletic Activity Card fee (admission for a student to all regular season home athletic events)
  • $5 Locker Use fee
  • $15 Commencement Ceremony fee (graduating seniors only)

Co-curricular Fees

  • $25 Academic Decathlon, Choir, Band, Robotics, Research Club, Yearbook, STUGO

Extra-curricular Fees

  • $50 Jazz Band, Key Club, Art Club, SADD, Band Fronts, Chess Club

Grant-Funded Extracurricular Activities

  • $5 Teen Court, Skills USA, FCCLA

Fine Arts Supply Fee

  • $50 Per Year - Pre AP Art, AP Studio Art, AP Art History, Sculpture/3D  Design, Basic Design
  • $25 Per Semester - Drawing & Painting, Ceramics/3D Design, Ceramics/3D Design II, Printmaking

Practical Arts Supply Fees

  • $50 Elements of Design, Graphic Comm. Fund, Graphic Comm. App, Photography

Science

  • $30 Science class supply fee (all science classes)

Physical Education

  • $15 Lifetime sports fee (bowling unit in class)

Fall Sports Fee Only

  • $150 Football, Volleyball, Cross Country, Cheer

Performing Arts Fees - Paid directly to the Music Director

  • $100 Band School Instrument Fee 
  • $70 Choir Apparel Fee
  • $50 Band Apparel Fee
  • $15 Band Supply fee (Flip Folder)

Refund Policy: 

Fees are refundable, with the exception of Sports fees and Club fees.
